Mackay Hospital and Health Service (Mackay HHS) commissioned a Health Service Investigation on 28 October 2021 into obstetrics and gynaecology services within Mackay HHS.
The investigation team of Dr Greg Duncombe, Professor Edward Weaver, Professor Mary Sidebotham and Leah Hardiman were tasked with reviewing:
- community and staff feedback about the provision of obstetrics and gynaecology services
- recorded incidents and complaints between 1 July 2019 and 31 October 2021
- any other data recorded by Mackay HHS concerning the safety or quality of obstetrics and gynaecology services between 1 July 2019 and 31 October 2021.

Multiple channels were available for patients, staff and other community members to provide information and feedback to the investigation team, including a dedicated hotline and email address.
Mackay HHS also established a consumer case management pathway for women who provided feedback about their obstetrics and gynaecology care. It was an opportunity for any women to have their medical care externally reviewed by an obstetrician and gynaecologist, and midwife (if appropriate).
Final report
The investigation team delivered their final report to Mackay HHS on 8 September 2022.
