Health Provider Portal

You can access your patient's health records on The Viewer through the Health Provider Portal (HPP) website. If you don't have access, you can register on the Queensland Health website.

Records include:

  • pathology reports
  • discharge summaries
  • medical imaging reports
  • medicine lists
  • outpatient letters
  • operation reports
  • test results.

You may also be able to access your patient's records if they have a My Health Record. Learn more about My Health Record on the Australian Digital Health Agency's website.

Our information access unit

You can request access to patient health records from our information access unit.

You need written consent from your patient to access their records. You’ll also need to include their:

  • name
  • date of birth
  • contact details.

You'll need to be specific about the health record you need. For example, a copy of their colonoscopy report in 2018. If your request is for many records, we may contact you to reduce the scope of your request.
