- share data to support better planning and delivery
- improve communication between GPs and hospitals
- connect information systems across healthcare
- make it easier for people to get the care they need
- test and improve new ways of delivering care
- focus on prevention and early treatment
- support healthcare workers in all settings
- involve patients and clinicians in improving services
- improve health outcomes for disadvantaged groups, including First Nations people.
