  • share data to support better planning and delivery
  • improve communication between GPs and hospitals
  • connect information systems across healthcare
  • make it easier for people to get the care they need
  • test and improve new ways of delivering care
  • focus on prevention and early treatment
  • support healthcare workers in all settings
  • involve patients and clinicians in improving services
  • improve health outcomes for disadvantaged groups, including First Nations people.
